Enrollment at CHS

In order to enroll your son/daughter at CHS a few important documents are needed at the time of registration:
 
  • Photocopy of driver's license of person enrolling student
  • Proof of residence (examples below)
    • Utility Statement
    • Voter registration
    • Notarized Affidavit
  • Copy of student's social security card
  • Copy of student birth certificate (please note: the birth certificate must be an official copy, not hospital)
  • Immunization record
  • Last report card/withdrawal form
 
In addition to the above documents, all parents are required to fill out a new student enrollment packet, which is available for pick up at Columbus ISD Central Office. As soon as all requested documents are received, we will work with you and your student to create a schedule of classes so your student can begin school.
